New End-of Life Document a Resource for Physicians, Patients

BATON ROUGE- The LaPOST document, recently given final approval by the Louisiana Legislature, is now available as a physician resource when helping patients plan and state their end-of-life treatment preferences and goals of care.

The Louisiana Physician Order for Scope of Treatment specifies patients’ wishes in the event they are unable to communicate. It is a physician order and is only recommended for patients with a life-limiting illness and a life expectancy of less than one year.  Physicians and their patients must complete and sign the LaPOST document for it to be valid.

The document is one of several publicly available resources for end-of-life planning available on the LaPOST website, www.la-post.org.  The website provides instructions on completing the form, examples of LaPOST usage in a variety of medical settings, and recommendations for ensuring information in the document is accessible to all health care professionals treating the patient. The site also provides explanations of advance directives and health care power of attorney documents.

LaPOST was created as a best-practice model for advance-care planning through the efforts of the LaPOST Coalition, a statewide network of Louisiana health care professionals. It was recommended by the End of Life Work Group of the Louisiana Health Care Redesign Collaborative and endorsed by the Louisiana State Medical Society.

Composed with input from health care and legal professionals, the document is completely voluntary and neither for nor against life-sustaining treatment. However, LaPOST is a medical order. Once completed, all health care professionals must comply with the specified treatment.

LaPOST Coalition Chairwoman Dr. Susan Nelson says a key benefit is that the document transfers with patients across health care settings - from a hospital to a nursing home to a hospice, for example.

“LaPOST was created to help Louisiana’s health care system honor and support patients’ end-of-life wishes,” says Nelson, medical director of senior services for the Franciscan Missionaries of Our Lady Health System. “The resources available on the LaPOST website are meant to assist physicians in having this conversation with patients and their families and complete and implement the form. Health care professionals are

encouraged to check our website for more information on end-of-life care that will be added in the coming months.”

The official form is printed on gold-colored paper for easy identification and includes a LaPOST watermark visible on originals as well as photocopies.

The LaPOST Coalition is organized through the Louisiana Health Care Quality Forum, a private nonprofit group focused on collaborative health care initiatives that benefit all Louisiana residents, including a standardized system for electronic medical records. In addition to health-information technology, LHCQF focuses on quality measurement, clinical quality improvement, medical homes, outreach and education. Board members are volunteers who represent a cross section of public and private insurance purchasers, patient advocates, providers, physicians and insurers throughout the state. More information is available at www.lhcqf.org.

Special Art Pieces Donated To Memorial Hospital Psychiatric Services Department

Lake Charles —Specialized art pieces are now on permanent display in the Lake Charles Memorial Hospital Inpatient Psychiatric Unit, thanks to the owners of Signs Now, a Lake Charles company specializing in computer-designed signs, and Dale Archer, M.D., local psychiatrist with the Institute for Neuropsychiatry and member of the Memorial Hospital Medical Staff.

The art project, valued at over $3,000, features seven panels of gorgeous scenic photography.  The pieces are made of vinyl and look as though they are professionally framed.  The project is part of the hospital’s Art for the Soul program.

“Before now, due to safety precautions, we were unable to place framed art pieces within the inpatient psychiatric patient care area,” says Leif Pedersen, Senior Vice President of Philanthropy.  “Signs Now, along with Dr. Dale Archer, came up with a secure and harmless alternative.  Thanks to their generous contributions and hard work, this project is now a reality.”

Mercy Regional Medical Center & Acadian Medical Center Earn “Top Performer on Key Quality Measures” Recognition from The Joint Commission

(Ville Platte & Eunice, LA) – Mercy Regional Medical Center, along with its Eunice campus, Acadian Medical Center, has been named one of the nation’s top performers on key quality measures by The Joint Commission, the leading accreditor of healthcare organizations in America.  The hospitals were recognized based on data reported on evidence-based clinical processes that have been shown to improve care for certain conditions, including heart attack, heart failure, pneumonia, surgical care and children’s asthma.

Mercy Regional Medical Center is among only 405 U.S. acute care and critical access hospitals earning the distinction of “Top Performer” on key quality measures for attaining and sustaining excellence in accountability measure performance.  Inclusion on the list is based on an aggregation of accountability measure data reported to The Joint Commission during the previous calendar year (i.e. the first recognition program is based on data reported for 2010).

To be recognized as a Top Performer on key quaility measures, an organization must meet two 95 percent performance thresholds.  First, they must achieve a composite performance of 95 percent or above after results of all of the accountability measures for which they report data to The Joint Commission were factored into a single score, including measures that had less than 30 eligible cases or patients.  Second, they must meet or exceed a 95 percent performance target for every single accountability measure for which they report data, excluding any measures with less than 30 eligible cases or patients.

Mercy Regional Medical Center was recognized for achieving these thresholds in the categories of:  Heart Failure, Pneumonia and Surgical Care.

“Today, the public expects transparency in the reporting of performance at the hospitals where they receive care, and The Joint Commission is shinning a light on the top performing hospitals, such as Mercy Regional Medical Center and Acadian Medical Center, that have achieved excellence on the number of vital measures of quality of care,” says  Mark R. Chassin, M.D., FACP, M.P.P., M. P.H., president, The Joint Commission.
